In your case I wouldn't start with your manners but your dissociative identity disorder (DID).
A quick research has taught me that Individuals with DID often also suffer from other mental illnesses, including posttraumatic stress disorder, borderline and other personality disorders, and conversion disorder. So your manners and your arrogance might be the least important of your problems.
The recommended treatment is psychotherapy but in a grievous case like yours I'd try an electroconvulsive therapy.
