Sword spec can't proc when you are not auto-attacking target affected by your ability.
Tested with wirlwind (50 times), and this hamstring (200 times) macro:

/stopattack
#showtooltip
/stopattack
/cast Hamstring
/stopattack

I saw atleast twice sword spec animation (black curved lines moving near hand) from this macro, but there was no combat log text or attack to follow(normally it works with hamstring)
I suspect this bug also applies to rouges sword spec and other abilities, but I haven't tested.

There is another bug related, sometimes you can see the animation, yet there is no attack to follow.

I suspect it is cause sword spec here checks if you are facing your target, are you in range etc, normally it should be enough if you were in range(or whatever) with the attack triggering the sword spec, sword spec itself should not check this anymore.
It seems that there is a short delay between the triggering attack and the sword spec attack, you don't even need to have lag to fail with the sword spec, while lagging this probably occours more often.

So for example is never happens when you and your target are standing still, but happens from time to time if you both are running around eachother.

I'm not talking here about dodges/parries/misses, but that the sword spec fails to occour.

I suspect the sword spec here uses same mechanic as auto attack, thats why I see such bugs.
