Compared to other expansions, it seems like it's quite hard to get your rage up and sustain it.

What's the best way to keep it up?
Usually I try to get it up by auto attacking and such but it seems pretty slow to do it that way.
Any suggestions?

rage generating is low here.check the other TBC servers and you will see

To generate rage u must increase your expertise in hit ratings , basically it means dont go for such avoidance , just keep your def cap which is basically around 500 (490 cap) and use proper enchants and gems . Playing warrior tank here on Smolderforge is hardcore , because armor of all creatures is higher then it should be , and rage gen from your attacks is less then original , so less rage .

Protip, hitting Target Dummies in the Battlemasters are does not give you the correct amount of rage.
